Instructions for Form

Please complete all fields required for your entry. 

Team captain information is required. This is the person that will receive all corrospondance and tourncment checks.

Team Roster - enter all information for all bowlers on this entry, this includes their USBC ID number. In this section please check for each bowler if they are entering all events. Women can enter either the Women's or the Open, but not both.

After completing this page click on the NEXT button to team rosters, doubles and singles and select squad times. Must clcik the box for each event you are entering bowlers, this will display the information required.

Once you are satisfied with your entry, click on the NEXT this will take you to an order summary page and payment options. Choose your option and complete payment, no entry will be accepted without propoer payment. Once that is completed, you will receive confirmation emails on your payment and entry, and so will the Seniors Tournament Director.
